Do You Actually Need a Mattress Topper or Encasement?

I used to think mattress protectors were one of those "nice to have, never actually needed" purchases. Then a bad spill happened on a mattress that wasn't cheap, and I changed my mind fast.

Quick terminology note before we get into it: what most people call a "mattress topper," a thick foam or latex layer for comfort, is actually a different product from what's in my Room Refresh collection. A topper changes how the mattress feels. A protector or encasement protects what's already underneath. If you're asking "do I need a mattress topper" but you're really picturing spill protection or allergy relief, here's what you're actually choosing between.

The two options, and what they actually do

A mattress pad (what I carry as the Waterproof Quilted Mattress Pad) fits over the top of your mattress like a fitted sheet. It covers the sleeping surface with a waterproof layer plus a bit of quilted cushioning, and stops there. The sides and bottom of the mattress stay uncovered.

A mattress encasement (my Waterproof Zippered Mattress Encasement) wraps the entire mattress, top, sides, and bottom, and zips shut. Nothing gets in or out except through that seam.

According to Sleep Foundation, both styles reduce moisture buildup, allergens, and stains, and both help preserve your mattress warranty, since a lot of warranties get voided the moment a mattress shows a stain. The real difference is how much of the mattress is actually sealed off. Mattress Nerd's comparison is blunt about it: protectors are fine for occasional accidents, but if bed bugs, allergies, or frequent big spills are the actual concern, only a full encasement seals the mattress well enough to matter. Dust mites and bed bugs can still get in and out of a top-only pad through the exposed sides.

So which one do you actually need?

Honestly, it depends on what you're protecting against.

  • Kids' beds, dorm beds, guest rooms: a pad is usually enough. You're guarding against spills and general wear, not allergies.
  • Dust mite allergies or asthma: the encasement will meaningfully cut the allergen itself, that part is well documented. Whether it actually calms your symptoms is a murkier question than most product pages let on, more on that below.
  • Moving into a new place, buying secondhand furniture, or just being cautious about bed bugs: encasement, no debate. A pad that only covers the top surface won't stop anything from getting in through the sides.
  • Postpartum, pregnancy, or general peace of mind overnight: either works. The pad is the more affordable, easier-to-wash option for this one.

What I'd actually tell you to buy

If you're outfitting a dorm bed or a kid's room, the Waterproof Quilted Mattress Pad is the one I'd start with. Thirty dollars, machine washable, and it doesn't feel like sleeping on a pool float, the quilted top is genuinely comfortable, not the crinkly plastic-sheet feeling you might be picturing.

If allergies, bed bugs, or a bigger spill risk are actually part of the conversation, spend the extra ten dollars and get the full encasement instead. It's the only one of the two that actually seals the mattress.

The nerdy version, for anyone who wants the actual research

I got curious enough to go pull the actual studies instead of just trusting product-page marketing, mine included. Here's the honest picture, which is a bit more complicated than "encasement, therefore no more allergies."

Encasements genuinely do the one thing they're mechanically built to do: block allergen from crossing the fabric. A controlled trial in Annals of Allergy, Asthma & Immunology found that even partial mattress encasing significantly reduced dust mite antigen measured on the bed sheet surface. A meta-analysis pooling 24 trials found impermeable covers produced a real, statistically significant drop in dust mite allergen levels inside the mattress.

Here's the part that surprised me. That allergen reduction doesn't reliably translate into fewer symptoms. The same meta-analysis found no significant improvement in asthma, wheeze, allergic rhinitis, or dermatitis from using covers alone, across both prevention and treatment settings. A 12-month randomized, placebo-controlled trial in adult asthmatics found this directly: allergen levels dropped substantially in the treatment group, but "there was no effect on peak flow or asthma symptoms" compared to placebo. The researchers' own conclusion was that allergen-impermeable bedding on its own "cannot be recommended as an effective way of relieving asthma symptoms."

There's an even stranger finding buried in one study run in a tropical climate: fully encased mattresses actually saw mite levels increase inside the mattress over six months, while partially encased ones saw a decrease, likely a humidity and airflow effect specific to that climate. I wouldn't assume that result carries over to an Ontario bedroom, but it's a good reminder the research isn't as tidy as "more encasement, always fewer mites."

None of this touches the bed bug case. Physically sealing a mattress is still the most reliable barrier against something crawling in or out, that's a mechanical seal, not an immune-symptom question, so the studies above don't really apply there. An encasement also still blocks spills, stains, and the kind of mattress damage that voids a warranty, regardless of what it does or doesn't do for your sinuses.

So the honest version of my advice: buy the encasement for bed bugs, spills, warranty protection, or general peace of mind. Don't buy it expecting it alone to fix your allergies, the actual research says it probably won't, at least not by itself, without other measures like regular vacuuming or humidity control alongside it.
